 The basic event in diabetes is the rise in blood sugar and the spilling of sugar (glucose) in the urine. When sugar is excreted in the urine, urine volume rises because more water is needed to dissolve the abnormal sugar load; hence, the two characteristic symptoms - increased frequency of urination and excessive thirst. A screening test for diabetes is a check for urine glucose - this can be done simply and cheaply with a dipstick - a color change on the indicator pad reveals the presence of sugar. Sugar is not detectable in normal urine. The diagnosis is confirmed by finding elevated blood glucose levels especially in the morning before eating. A measurement of long-term blood sugar levels (glycosolated hemoglobin) is helpful to confirm the diagnosis. The glucose tolerance test is seldom required to make the diagnosis and should be used only special circumstances.

Most people know that insulin is used to treat diabetes and will assume that insulin deficiency causes all diabetes. Unfortunately, the problem is not that simple. Only 10 % of diabetics lack insulin and the rest have a different disorder or a range of disorders that lead to elevated blood glucose and poor utilization of glucose.

Impact of Diabetes: Diabetes 2 is a name for an evolving complex of metabolic derangement's that contribute to a horrendous list of chronic and disabling illnesses. While elevated levels of sugar in the body seem to be the leading edge of the emerging disaster, changes in fat metabolism, liver and kidney function and then circulation impairment add to a cascade of dysfunction. 75% of the early deaths in diabetics are related to coronary artery disease.

According to the National Diabetes Information Clearinghouse (a service of the National Institute of Diabetes and Digestive and Kidney Diseases, U.S. Public Health Service.): Diabetes is widely recognized as one of the leading causes of death and disability contributing to the deaths in the USA alone of more than 169,000 persons in 1992. Standard medical treatment protocols for Diabetes 2 always mention diet revision and then quickly proceed to medication options. While diet control is always mentioned , the critical, decisive importance of diet revision and exercise is not emphasized and in practice, diet revision is often neglected in favor of drug treatments.

The management of diabetes has been dominated dietetic thinking in this century and nutritional dogma prevails in the management of diabetics. Most diabetics are taught to use exchange diets that target a daily calorie intake and distribute the calories among the four food groups.
